Create a patient account for E-LOCATE
Last updated: October 2026
E-LOCATE is a Patient ITR Locator & Scheduling System run by the hospital operating E-LOCATE. By creating an account or logging in, you confirm that you have read and agree to these Terms of Use and the Privacy Policy below. If you do not agree, please do not use the system.
E-LOCATE helps you find where a record is and what its status is. It is a guide only and does not replace the hospital staff, official records, or professional medical advice. If something looks wrong, please confirm it with the hospital personnel.
The admin may suspend or remove an account that breaks these terms or puts patient information at risk.
These terms may be updated from time to time. Continuing to use E-LOCATE after an update means you accept the new version.
Your information is available only to authorized hospital personnel and administrators who need it for their work. Activity logs are visible to the admin. Your information is not sold or used for advertising.
Accounts are protected by passwords and role-based access, so each user only sees what their role allows. No system is completely risk-free, so please log out on shared devices and never share your password.
Records and activity logs are kept only as long as needed for hospital operations, security, and any legal or record-keeping requirements.
Under the Data Privacy Act of 2012 (Republic Act No. 10173), you have the right to be informed about how your personal data is used, to access it, to correct inaccurate information, and to object to or ask for the removal of your data, subject to the hospital's legal duties to keep medical records. Health-related information is treated as sensitive personal information and handled with extra care.
To ask about your data or make a request, please contact the E-LOCATE system administrator.
Allow E-LOCATE to send you notifications. When you say yes, your device location will be turned on too, so your activity log shows where you signed up from.
Your device will ask you to allow notifications, then to share your location. Choose Precise so your exact spot is detected. You can still register if you say no.
We couldn't get your location. Your phone's Location (GPS) is probably switched off.
Turn it on from your phone's quick settings (swipe down from the top of the screen) or in Settings > Location, then tap Try again.
